    Responsibilities

    Under the direct supervision of the Manager of Support Services & SNP Reporting, this position provides administrative support for the department. Duties include staff record-keeping, updates in Kronos, record processing into the care management system, coordination of meetings and conferences, faxing, filing and scheduling, obtaining supplies, and working on special auditing projects. The Coordinated Care Assistant ensures that all assignments and administrative tasks are completed in a timely manner to reach departmental goals, and maintains good communication between staff and management. Able to work independently and efficiently to plan, prioritize, and organize a diverse workload and to recommend changes in office practices or procedures.

    Qualifications

    • Education HS Diploma or equivalent, required; College Degree preferred
    • Experience 1 Year experience in Health Care Environment Managed Long Term Care/Utilization management experience preferred
    • Specialized Knowledge and Skills, License/Certification
    • Excellent written and oral communication skills
    • Excellent computer and technology skills
    • Self-motivated and ability to initiate critical work tasks with minimal oversight
    • Ability to be empathetic and understanding with appropriate professional boundaries and problem resolution focus
    • Ability to work closely with different professionals as part of an interdisciplinary health care team
    • Ability to set priorities and meet deadlines
    • Appropriately identify and allocate resources, maximizes efficiency and contain costs
    • Ability to work independently with capacity to identify and communicate critical issues that have departmental or organizational impact and require peer, supervisory, or management input
