    Through analysis of health risk assessment tools, claim, encounter and HEIDIS data, identify and target opportunities for population health initiatives. Research available resources, implement and evaluate outcomes. Collaborate with Medical Mgt programs and act as a resource for health education and preventive service initiatives across the Elderplan population.


    Bachelor's degree in Public Health, Health Administration or related field or BSN. Master's a plus. 1-2 years experience in managed care. Will consider a combination of experience and education. Successful program development and/or project management experience, proficiency in data analysis and principles of population health management required. Proficiency in database operations, software systems and internet research skills also needed.
