Responsibilities

The Team Manager provides leadership guidance to the professional staff on a day-to-day basis to ensure the provision of Hospice services are consistent with the philosophy, goals, and policies of the program and MJHS.

 

  • Monitors and evaluates the quality and completion of nursing documentation per the documentation policy and procedure and state and federal regulations.
  • Facilitates and oversees the preparation and planning of the weekly IDT team meeting to ensure that all patients are reviewed per the Hospice Conditions of Participation (COP’s).
  • Ensures the implementation and ongoing evaluation of the Hospice Performance Improvement Plan related to NHPCO Family Satisfaction Survey results related to assigned clinical area with direct reports.
  • Participates in referral and community education and presentations to enhance hospice awareness.
  • Integrates department services into the organization’s strategic mission by setting goals that align with the mission and monitoring progress toward goals.
  • Manages performance of direct and indirect reports. Provides orientation, coaching, training and development. Takes corrective action when necessary.
  • Utilizes corporate services resources such as Human Resources, Compliance, Legal, Finance, Information Services, Marketing, etc., as appropriate.
  • Ensures appropriate staffing and resources to support department/agency services. Monitors productivity throughout the year and participates in annual budget preparation.
  • Interviews and participates in the selection process for qualified staff in collaboration with Human Resources and department management.

Qualifications

  • Graduate from an accredited School of Nursing, BSN or BS degree preferred.
  • Minimum of five years of Hospice nursing experience.
  • Supervisory or leadership experience preferred.
  • New York State RN licensure required.
  • National Board Certification in Hospice and Palliative Care (CHPN) preferred or obtains certification within one year of hire.
  • Word, Outlook, and Windows experience required.
  • Ability to effectively supervise and mentor Hospice nursing staff.
  • Ability to work effectively within an interdisciplinary team environment.
