MJHS Home Care provides advanced clinical services and emotional support to patients recovering from illness, injury, or surgery. With a comprehensive range of services supported by an integrated health system, patients can recover in the familiar surroundings of their home.
In this role, you will valuate patients referred for home health services. You will manage and facilitate the transition of patients onto the home care program in accordance with applicable law, regulations and agency policies and procedures.
Provides flexibility within the role of effectively supporting the agency’s overall business objectives and goals
Establishes a plan of care (POC) in collaboration with patients, physicians and members of interdisciplinary health care team and arranges for all needed services under the direct supervision of an HCP Manager and/or Director of Business Development
Conducts an evaluation of referral needs to determine potential alignment with additional MJHS products and services ( Elderplan, HomeFirst, Hospice, Palliative Care); under the direct supervision of an HCP Manager and/or Director of Business Development
To identify and assess referrals received for the various MJHC clinical service programs (e.g. Heart Failure, COPD, Joint Replacement, Diabetes Management, Community Based Palliative Care) as deemed applicable; under the direct supervision of an HCP Manager and/or Director of Business Development
