Ensures review of all On-Call communication from primary members of the IDT, noting need to follow up with phone calls and visits. (On Call TM will determine who will make the calls). Mentors and provides leadership direction to all clinicians (Float and On-Call Nurses) of the assigned team. Graduate from an accredited School of Nursing, BSN or BS degree preferred. Minimum of five years of Hospice nursing experience. Supervisory or leadership experience preferred. Word, Outlook, and Windows experience required. New York State RN licensure, NYS valid license preferred. National Board Certification in Hospice & Palliative Care (CHPN) preferred or obtains certification within one year of hire.
