Overview

Per Diem Rates:

 

  • On Call - regular days Monday - Friday (shift 5pm - 8am). Payment is $215
  • On Call -Weekends and Holidays. The shifts are from Saturday 8am -Monday 8am (or morning after the holiday).  Shifts on weekends and holidays are 12 h (8am -8pm and 8pm -8am), payment is $240 /12h shift.
  • Weekend facility rounds or urgent cases overtime ( ISNP/IE-SNP ) -$ 90 /1hour ( usually 4 hours /facility )
  • Community Program MJHS Plus overtime -urgent Home Visit Initial /Chronic Care Comprehensive -$180, urgent home visit follows up visit -$150

Responsibilities

In collaboration with physician, provides and coordinates primary care services to an assigned panel of Advantage members who reside in participating long term care facilities.  Insures development of care planning and access to preventive care services in accordance with member and Plan health outcome goals and objectives.  Acts as clinical resource to the participating providers and facilities.  Acts as team member and clinical resource to participating providers/facilities.  Acts as resource and contact for member/designated representative regarding all aspects of member individualized care plan and disease process.

Qualifications

  • Masters Degree in Nursing, graduate of accredited Nurse Practitioner program required
  • One to two years clinical experience in role of nurse practitioner required
  • Minimum one year in geriatric specialty
  • Previous clinical RN experience in acute, subacute and/or long term care setting preferred
  • Prior experience in manage care a plus
  • NYS RN/NP License, Registration, DEA certificates
