Responsibilities

  • In collaboration with physician, the NP will be providing comprehensive assessment, including multifactorial complex physical examination and psychosocial needs, self - management education & tools to better manage patient’s multiple conditions
  • As a part of Interdisciplinary Co-Management Program, the NP will be providing and coordinating primary care services to an assigned the at-risk panel of Elderplan members who reside in the community
  • The NP will insure development of care planning and access to preventive care services in accordance with member and Plan health outcome goals and objectives
  • The NP will act as clinical resource to the member, family and interdisciplinary team.

Qualifications

  • One year of clinical experience in role of nurse practitioner required; Minimum one year in geriatric specialty
  • Previous clinical RN experience in acute, subacute and/or long term care setting preferred
  • Prior experience in managed care a plus
  • Ability to assess, diagnose, treat and follow-up
  • Ability to work independently and as part of interdisciplinary teams 
  • Ability to develop relationships and communicate to members/family 
  • Establish priorities and maintain schedules
  • NYS RN/NP License, Registration, DEA certificates
