Our range of health services include home care, hospice and palliative care for adults and children, rehabilitation and nursing care at Menorah and Isabella Centers, and the research based MJHS Institute for Innovation and Palliative Care. Responsibilities

MJHS Home Care provides advanced clinical services and emotional support to patients recovering from illness, injury or surgery. With a comprehensive range of services supported by an integrated health system, patients can recover in the familiar surroundings of their home.

 

  • Assesses patient psycho-social issues to ensure adequate delivery of home care services in order to meet agency goals and required objectives
  • Obtains psycho-social history and conducts evaluation and assessment of patient including adequacy of patient support system
  • Provides supportive counseling in regards to emotional, psychological and concrete needs including discharge planning
  • Interprets and provides information about available benefits, entitlements and community programs to patients and family
  • Counsels patients and receommends community based resources regarding their psycho-social needs

Qualifications

  • Master's degree in Social Work required
  • New York State Social Work License required (LMSW or LCSW)
  • Knowledge of the operations of community-based health and social services programs, concrete services and government entitlements required
  • Home Care experience preferred
  • Must be able to provide own transportation in order to make visits in assigned area(s) within greater NYC
